ICD起源于1925年在瑞士日内瓦举行的世界儿童健康会议。不清楚为什么6月1日被选为ICD：一个理论，中国在旧金山总领事（美国）收集了许多中国孤儿庆祝1925年端午节，恰好是在当年6月1日，也正好在日内瓦。
因为6月1日被许多国家视为ICD，特别是共产主义国；在西方世界，ICD通常在一年中的其他日子庆祝（如果有的话），而且公众对这些活动往往知之甚少。（进一步讨论见以下德国部分。）因此，有时会有一种误解，即6月1日的ICD是共产主义的发明。尽管如此，近年来，甚至是美国的一些团体也在6月1日开始观察ICD。
